developmental disabilities living in residential programs. Sets performance
standards and provides the support and training for staff to achieve standards.

experience and two years experience working with individuals with developmental

opportunities and/or activities that promote individualization, independency,
inclusion, and productivity. Responsible for coordinating services with agency

human services field, two years supervisory experience and two years experience
working with individuals with developmental disabilities. QMRP status.

Provides activities of daily living including personal care and program
activities for individuals with developmental disabilities living in community
residences. Maintains records and charts. Performs housekeeping duties.
Part-time including weekend positions available. Experience working with people
with disabilities preferred. Related Nurses Aide/CNA experience a plus.

Plans and implements prescribed hearing programs or the use of hearing devices
for consumers to maintain their current level of function secondary to acute or
chronic disability; conducts annual screens assessments, and daily
documentation if warranted. Masters degree in Audiology and a NYS License.

Applied Behavioral Sciences Specialist
Works in conjunction with the interdisciplinary team to assure that the mental
health and behavioral needs of the consumers are addressed. Responsible for
behavioral assessments and behavior plans; provides individual and group
counseling; trains staff. Masters Degree in Psychology and 1-2 years of
experience with persons with developmental disabilities preferred.

Nurse Case Managers
Responsibilities include nursing assessments, diagnoses, coordinating the
delivery of health care services for adults with disabilities residing in
residential programs. Works in conjunction with other members of the
interdisciplinary team to achieve positive health outcomes for consumers.
Registered Nurse with NYS License and one-year experience working with
individuals with developmental disabilities. Ability to travel to multiple
sites within an assigned borough and able to work flexible hours and/or days.
Bachelors Degree preferred.

Registered Nurse
Directly coordinates and delivers health care to residents living in residential
group home. Responsible for nursing assessments, writing of diagnoses,
developing and intervening in the patient's health care plan to ensure quality
care in the most efficient manner. Advocates for health and accesses hospital
and specialty clinical services. Full-time or Per Diem; various shifts
available. NYS Registered Nurse License, Experience with persons with
developmental disabilities preferred. CPR Certification.

The Charge Nurse will oversee the provision of services provided to patients by
healthcare center physicians, clinician's nurses and aides to ensure that all
health needs are addressed in an appropriate and efficient manner. NYS
License-Registered Nurse; Minimum three years nursing experience with one year
in a supervisory capacity. CPR Certification.

Assists in conducting regularly scheduled Medication Administration Courses and
refresher/remedial classes for direct care staff in Metro New York division
programs. Provides on-site training for R.N.'s conducting medication evaluation
practicums for direct care staff. Conducts Agency wide specialty training on
health related topics, as assigned. NYS Licensed Registered Nurse; Minimum two
years experience working with people with developmental disabilities. CPR
Certification.

Provides training and assistance to individuals with TBI in the areas of
residential habilitation and community living. Develops written ILST Plan that
focuses on the independent living needs and the chosen goals of the Program
Participant. Bachelor's degree in related field and a minimum of two years
experience providing functionally based assessment and training with
individuals with disabilities

Utilizing a person-centered approach to service planning and delivery, develops,
implements and maintains each person's Individual Service Plan (ISP). Provides
assistance and/or guidance in identifying and accessing those activities,
supports and services necessary for the individual to achieve his/her valued
outcomes. Bachelor's degree in related health services field and one year
experience working with individuals with developmental disabilities.

Habilitation Coordinator
Supervises, designs and monitors the implementation of individual treatment
plans for consumers attending Day Program. Prepares and reviews monthly notes,
maintains consumer's records, supervises annual conferences, semi-annual
reviews, and special team meetings. Bachelor's/Masters in related field; QMRP
Status and minimum of two years experience working with persons with
developmental disabilities.

Assists Day Program staff in the provision of active treatment services
including the development and implementation treatment plans for consumers in
day program and day habilitation settings. Completion of OMRDD Approved
